Valley Surgical Group has served the communities in the bay area for several years. We are affiliated with most Health Maintenance Organizations and Independent Physician Organizations in the area. You may read about our individual surgeons in Meet the Surgeons section.
Our Office Staff
Our office staff is always happy to assist you with any scheduling questions, as well as pre and post surgical arrangements. They will help with any insurance issues as well. The staff will assist with all pre-certification issues with your insurance carrier, managed care organization, Medicare or Medicaid. All claims will be filed for you and any supplemental policies will be submitted for you as well. Our staff prides itself on being friendly, helpful and timely. Please do not hesitate to call for any informational item. You may also email our billing manager with any questions about your bill.
FORMS
We have convenient online forms to help make your visits to our office easier.
New patients may download these forms and print them out, fill them in and bring them to the office. We hope that completing these forms at home will make it easier to give thoughtful and detailed answers to the questions, as well as shorten the time spent at the appointment.
Patients will also get the opportunity to consult the labels on the medication bottles and complete in a detailed and accurate manner the list of medications used. This information is vitally important to the surgeon when scheduling procedures and making other decisions on a patient's care.
The Medical Release form is for existing patients and only required if you need your medical records sent to another office.

Valley Surgical Group accepts the following insurance plans. We are also affliliated with most medical groups serving East Bay. Our office staff is happy to work with patients to ensure they receive adequate financial assistance to fund their care.
